Coping with Skin Picking or Scratching

Skin picking – whether it’s squeezing a spot here and there, or scratching off a scab – we’re all guilty of doing it at least once in our lives. But what happens when you start to obsessively pick your skin, spots, moles, and scars?

We’re talking about when you’re unable to stop, and it gets to the point where you’ve caused cuts, bleeding, bruising and/or scarring on the skin. You might spend hours each day picking, using your fingers, a pair of tweezers, pins or comedone extractors as a result of stress, anxiety, or even boredom.

You may even do this without realising it, and in some extreme cases, in your sleep. In this case, you might have Dermatillomania, also called ‘Skin Picking Disorder’ or ‘Excoriation Disorder’ which is where you cannot stop picking at your skin.

Skin experts, Cai Graham, Anxiety Support Specialist and Kimberley Medd Clinic Lead at Face the Future explain what is Dermatillomania and how to alleviate it from a skin care and mental health perspective.

Cai Graham explains, “Dermatillomania is often misdiagnosed as a ‘self-harm’ tactic, but this isn’t the case. Sufferers may do it to relieve anxiety, to delve into negative emotions like guilt, to ‘escape’ a situation they feel uncomfortable in, or even to cure boredom, as opposed to wanting to cause pain to themselves.

“It might also be due to compulsive perfectionism, which is the belief that skin picking will lead to ‘perfect’ skin.

“The disorder sees the sufferer entering a trance-like state in which they obsessively pick, pull, and/or scratch at their skin. This varies from the face, to the arms and chest, to calves, or even the scalp, among many other places on the body.

“They may target ‘healthy’ skin, but also ‘imperfections’, such as blemishes, scars, scabs, moles and freckles. When in this trance, time passes by extremely quickly. 30 minutes. An hour. Time passes in the blink of an eye without the sufferer even realising it.

“While the sufferer feels intense relief and control during the skin picking session, once they are out of their ‘trance’, they feel extreme shame and guilt for what they have just done.”

How Can You Help Someone With Dermatillomania?

“It can be difficult knowing how to support someone with Dermatillomania. While you may be tempted to remind them to ‘stop picking’, this can cause further mental anguish, as well as potential conflict within your relationship. Here’s how you can help a sufferer.

“First of all, avoid pointing the blame or shaming them for having Dermatillomania. As difficult as it can be, do not express anger at them. It is important to remember that it is an uncontrollable condition – it is not a decision, nor a bad habit, but rather a compulsion.

“Ensure that you provide unconditional support, and help the sufferer to reach out to their GP to discuss remedies, such as Cognitive Behavioural Therapy or medication. Further encourage them to keep their hands busy with something else, such as a stress ball, when they do get the urge to pick.

“Also, try to help them recognise their triggers. For example, are they most likely to pick when they’re cooking dinner? When they’re in a meeting? Or on public transport? Recognising these triggers can make it easier to manage the skin picking ‘sessions’ by implementing certain steps, such as wearing gloves, to minimise or prevent the damage.”

Kimberley Medd advises on how Dermatillomania can effect our skin, and how can we care for it effectively.

“While Dermatillomania is derived from a Mental Health condition, it does directly affect the skin. Continuous picking and pulling at the skin can cause deep scarring, as well as potential skin infections.

“In an ideal world, it would be recommended to not pick at your skin, however with debilitating conditions such as Dermatillomania, it really isn’t that simple. Here are my top tips for caring for your skin when you have Dermatillomania.

“After a picking session, wash your hands and clean the skin thoroughly with warm water. Pat dry with a clean towel and do cover any potential skin infections with a bandage.

“It’s crucial that you do not share items such as towels, flannels or razors with anyone else, as bacteria found on these items can cause or exacerbate an infection on wounded skin.

“In terms of skincare, I would highly recommend incorporating soothing, anti-inflammatory ingredients into your routine.

Silvina Neder is a model, influencer and pro-ageing champion. She has her own stunning hair care range, Silvina London, with products to nourish and hydrate grey hair and enhance natural silver shades.

In our fascinating interview, Silvina tells us how she became a model. Why she is proud of going grey. And gives her top tips on haircare and beauty to help you feel healthy, confident, look stunning, and feel fulfilled.

Can you tell us a bit about yourself and where you come from?

“I am born and raised in Argentina, with a career in market research for almost 30 years.

“In my 40s I decided to come to London as it was my dream. I loved the city ever since I visited in my 20s.”

I understand your career was initially in marketing and consumer insights. How did you get into this, and what did you learn from your work?

Silvina Neder

“I came to this through my education in statistics, but I wanted more of the human element so consumer insights was perfect for me. I had own company in Argentina, then I worked at an agency in London.

“The industry changed a lot with the invention of AI and being more data driven and I started to not enjoy it so much. I was in my 50s and my colleagues were mostly in their 20s and 30s, so it was a different culture for me.

“Through my work, I knew the insights, and it told me there wasn’t much out there for women with grey hair. It also taught me to listen to what women want, their fears, dreams, etc. I always pay attention to that, and this is core to my brand, Silvina London.”

Your 50s saw a dramatic career change and you became a model and influencer. How did this come about? Can you tell us a bit about what you do?

Silvina Neder

“I was looking to do something different and as I had my own business before I was always looking for entrepreneurship. Everywhere I went, people said they love my hair and look.

“I thought I have to do something with this, so I Googled ‘grey hair models’ and found agencies and sent them the photos I had for my website. That week I had my first job as a model!

“I started to get more bookings and I enjoyed it. In a few months I was fully booked, so I deleted my market research website and went ahead with this full time.”

What do you enjoy most about being a mature model? And how do you feel you are helping others?

“I enjoy that anyone can be a model now – there are no stereotypes. You can inspire people to be themselves.

“I get a lot of comments on social media about being an inspiration and empowering other women to embrace the grey and feel confident, and that makes me feel amazing.”

For many years you coloured your hair, and then one day you decided to give up the colouring and embrace the grey. Can you tell us more about this, and also why you made the change?

Silvina Neder

“I used to colour my hair every two weeks and I struggled with the time to book this in, so I couldn’t continue. I had been thinking about going grey for many years.

“Compared to Argentina, London is very accepting and progressive and I just decided to embrace the grey.”

How did you make the transition in a natural and stylish way? Who styled your hair and helped you with your look?

“Most women don’t go grey because they are scared of the transition. I loved the experience as the grey hair was so shiny and healthy. One day I made the decision and I didn’t look back.

“You have to stick with it. I started looking at what to do, and if I should bleach the rest of my hair. But many people did not advise this, and by then I had about 6cm of grey roots.

“I cut my hair so it didn’t look too bad. I think now you see many people going through this transition and people don’t bat an eyelid, it’s absolutely fine. I have more about the transition and tips on my website at www.silvinalondon.com

As we mature, hair can get thinner and lack lustre. What would you recommend as the best products to keep it looking full, healthy and shiny?

Silvina Neder

“Hair does get thinner but without using colour on the hair it is healthier. I launched Silvina London and both products have Black Oat Extract which helps growth, and many customers tell me their hair feels thicker, healthier and shinier through using them.”

How often should we wash our hair to maintain its condition? What is the best shampoo and conditioner to use? And how should we massage it and work it into the scalp and hair for the best results?

“I wash my hair every single day as I do sports. If you use good quality products, you can wash it every day.

“I of course use Silvina London shampoo and conditioner. I don’t use anything else. If it feels a bit drier, I leave a bit of the conditioner on, but that’s it.”

You have your own haircare range. How did you create it? What is your ethos? And what is included in the collection?

“I created Silvina London because I couldn’t find anything good for silver hair. A lot of shampoos are chemical and colour the hair too much – they leave a bit of a blue/purple tinge to the hair. My shampoo uses naturally-derived pigments like Blue Magnolia, so you can use it every wash and the brassiness goes, and there is no tint to the hair.

“At the moment, I have a shampoo and conditioner in the range. I will be launching something else in a couple of months! My ethos is to have the best ingredients, give everything grey hair needs, be authentic and real. And keep the hair routine simple, we don’t need to use a lot of products.”

Having a good diet is also the key to healthy looking hair. What would you recommend?

“It’s all about moderation. Enjoying ourselves but not to excess. It is good to have a balance and enjoy life at this age, but we will look better and feel better if we have a good diet.

“Exercise is also very important for me, I do yoga, pilates and row. I enjoy it and it keeps me healthy and active.”

Daily skin care is important as we get older. I find that using a good moisturiser for day and night is essential. Which products would you recommend that are gentle, nourishing and kind to the skin?

Silvina Neder

“I use a good serum, night oil, and do face yoga – I believe you need to tone the muscles in the face like you do with the rest of your body. It is also important to remove all your make-up with a good cleanser. I really like Made of More skincare and Inlight Beauty skincare which is 100% organic.”

Foundation is a good way to even out skin tone and cover up blemishes. Can you suggest any good value products and how to use them?

“I keep my makeup very simple. A light foundation, concealer and mascara but not anymore. I don’t like a lot of makeup.

Turning 60 can often be a challenging time. Many of us lose confidence and struggle with hair, makeup and clothes. What would you recommend to help people feel good about themselves?

Silvina Neder

“I would recommend being as active as possible to stay strong. Establish a good skincare routine, take care of yourself, use sunscreen, wear your hair naturally, and use good quality products.”

Where do you like to go shopping for your clothes? And what would you say is your fashion style?

Silvina Neder

“My style is definitely to keep things simple and comfortable. This equals elegance to me. I usually wear flat shoes and oversized clothing.

“I buy less clothes, but better quality that last. I usually keep colours simple. I wear a lot of black, white, beige and grey!”

How do you stay feeling confident, content and happy. What is your secret?

“Having a purpose, something to wake up for and being creative. If you are creative it keeps your mind active.”
